#2258: lb: add timers for lb proper and migration

Closes #2258.

I used terse debug prints for phase, not the LB manager, so they get turned on when the post-LB statistics prints get turned on. Turning them on with the LB manager would have added a bunch of other prints as well, which is not desirable.
