Locale prefixes should be applied to all URLs, not just page URLs

The current setup requires awkward workarounds like the Apostrophe-Locale header and has led to problems for enterprise clients. Figure out a more global way to do this, despite the awkwardness of requiring helpers to generate these URLs across the board, or determine that this cost is truly greater than that of requiring the header.
