Make sure cohortId argument is being used consistently #465

To do:

go through all the function where we have the cohortId argument and make sure it is being used
ensure that there is only a filter on cohort ID when it differs from all cohort ID in settings
check consistency of use of cohort ID / make it clear for what functions it means only applying the function to leaving the other cohorts unchanged vs dropping the other cohorts
